The Gut-Brain Axis

Ninety percent of the body's serotonin is produced in the gut. A clinical trial put people on a better diet and matched antidepressant trial outcomes. This is not fringe science. It just hasn't found its marketing budget yet.
The conversation about food and mental health has, for most of medicine's history, run in one direction only: mental illness affects eating, produces eating disorders, disrupts appetite, causes people to eat in patterns that their psychiatrists note with concern. The idea that the relationship also runs the other way — that what you eat affects your mental health not just incidentally but mechanistically, through specific, identifiable biological pathways — is a relatively recent arrival in mainstream psychiatric thinking, and it has arrived with considerably more supporting evidence than the psychiatric establishment was initially prepared to receive.
Nutritional psychiatry is now a recognised subspecialty with its own journals, clinical trials, and research centres. And the findings coming out of it are consistent enough, and surprising enough, that they deserve more public attention than they currently receive — which is largely crowded out by the louder, more commercially driven discourse about physical outcomes like weight and cardiovascular risk, even though the brain consumes twenty percent of your body's energy and is arguably the organ most sensitive to what you feed it.
The Gut-Brain Axis Is Not a Metaphor
The gut and the brain are in constant, bidirectional communication through a network of neural, hormonal, and immunological pathways collectively called the gut-brain axis. The vagus nerve — the longest cranial nerve in the body, running from the brainstem to the abdomen — carries signals in both directions, with the majority of traffic, counterintuitively, running from gut to brain rather than the other way around. Your gut contains more neurons than your spinal cord. It earns the nickname "the second brain" with some physiological justification. It produces roughly ninety percent of the body's serotonin — a neurotransmitter most people associate with mood but whose primary location and primary functions are in the gastrointestinal tract, where it coordinates intestinal movement. The bacteria in your gut synthesise neuroactive compounds, communicate with the immune system in ways that affect neuroinflammation, and produce short-chain fatty acids from fermented fibre whose effects on brain function are only beginning to be characterised.
When the gut microbiome is diverse and well-fed — by the fibre and polyphenols we covered in previous instalments — this communication system appears to support emotional regulation, stress resilience, and cognitive function in ways measurable in both animal models and, increasingly, human trials. When it is disrupted — by low-fibre diets, ultra-processed food, chronic stress, or repeated antibiotic courses — the downstream neurological effects include increased anxiety, depressive symptoms, and cognitive impairment in ways suggesting the gut is doing meaningful mental health work the brain cannot fully compensate for.

What the Clinical Trials Show
The SMILES trial — a landmark 2017 randomised controlled trial from Australia — assigned adults with moderate-to-severe depression either to dietary counselling toward a Mediterranean-style diet or to social support, and found that the dietary intervention group had significantly greater reductions in depression scores, with thirty-two percent achieving remission compared with eight percent in the control group. The effect size was comparable to antidepressant medication trials, achieved through food rather than pharmacology. Subsequent trials have largely replicated the direction, if not always the magnitude, of the finding. A meta-analysis in Molecular Psychiatry found that dietary quality was consistently associated with depression risk across large observational studies, with higher-quality diets associated with twenty to thirty percent lower risk.
The omega-3 fatty acid evidence deserves specific mention: EPA and DHA, the long-chain omega-3s found in fatty fish, have accumulated reasonably strong evidence for mood support and are consistently associated with lower depression risk in observational data, with some randomised trial support for clinical depression at therapeutic doses. The mechanism involves multiple pathways including anti-inflammatory signalling in the brain, neurotransmitter function, and neuroplasticity — the brain's capacity to form new connections, which turns out to be omega-3 sensitive in ways that antidepressant medication actually shares as a mechanism, which is a striking convergence of nutritional and pharmaceutical biology that the public conversation on mental health has been slow to absorb.
The Inflammation Connection
One of the most compelling threads in nutritional psychiatry is the inflammation-depression link. Chronic low-grade inflammation — the kind associated with ultra-processed food diets, sedentary behaviour, poor sleep, and gut dysbiosis — consistently appears in elevated levels in people with depression, and growing evidence suggests inflammation is not merely a correlate but a causal contributor: inflammatory cytokines cross the blood-brain barrier, alter neurotransmitter metabolism, and directly affect the brain regions involved in mood regulation. Anti-inflammatory diets — and the Mediterranean pattern is one of the most comprehensively characterised anti-inflammatory dietary approaches available — appear to reduce these markers in ways that may partially explain the mood benefits observed in trials. The chain from what you eat, to what your gut produces, to what your immune system releases, to what your brain experiences as mood is becoming difficult to dismiss as coincidence and increasingly hard to ignore as a clinical target. Your brain is not floating in isolation. It is downstream of everything else. Feed the upstream well.
Feed Your Brain Through Your Gut
Eat fatty fish twice a week for the omega-3s. Eat a wide variety of plants for the fibre your gut bacteria need to produce the compounds your brain runs on. Minimise ultra-processed food for the inflammation reduction your neurons will notice. The brain you are trying to support is downstream of the gut you are feeding — and the best available evidence suggests that feeding it well is one of the more powerful mental health interventions currently available without a prescription.
