You have been given the number. You have been told whether it is good or bad. What you probably weren't given is the context that would make it actually useful — and without that context, it is doing less diagnostic work than you think.

You have probably had your cholesterol checked. You have probably been given a number — or several numbers — and told whether they are good or bad, possibly with a recommendation to eat less saturated fat or consider a statin, depending on what those numbers looked like and how much time your doctor had available. What you almost certainly were not given, because a standard clinical appointment has neither the time nor the visual aids for it, is an explanation of what cholesterol actually is, what the numbers you were handed actually measure, why the relationship between dietary cholesterol and blood cholesterol is less straightforward than the 1980s suggested, and why two people with the same total cholesterol number can have dramatically different cardiovascular risk profiles depending on information that number alone does not capture.

Cholesterol is one of the most talked-about and least understood topics in mainstream health, which is a genuine problem — because understanding it with slightly more precision than "high bad, low good" turns out to have real implications for what you eat, whether you need medication, and how you interpret the lab report handed to you once a year with insufficient context.

What Cholesterol Actually Is

Cholesterol is not a dietary poison that snuck into your bloodstream through a poorly chosen breakfast. It is a waxy, fat-like substance your body produces endogenously — your liver manufactures the majority of the cholesterol in your blood, largely independent of how much dietary cholesterol you consume — and it performs essential biological functions: structural component of every cell membrane in your body, precursor to steroid hormones including testosterone, oestrogen, and cortisol, precursor to vitamin D, and raw material for bile acid synthesis. You cannot live without cholesterol. The question was never whether to have it. The question is about what form it is transported in and in what quantities.

LDL and HDL are not, technically, cholesterol. They are lipoproteins — protein-fat transport vehicles — that carry cholesterol through the bloodstream. LDL delivers cholesterol from the liver to peripheral tissues; HDL transports it back to the liver for processing or excretion. The shorthand of "LDL bad, HDL good" captures something real — elevated LDL-C is consistently associated with cardiovascular risk, and higher HDL-C with lower risk — but it loses critical nuance that a standard lipid panel does not provide without additional testing.

Your liver makes most of the cholesterol in your blood. The egg you ate contributed far less than the saturated fat that influences how efficiently your liver clears the LDL it produced itself. This distinction was absent from thirty years of dietary advice.

The Dietary Cholesterol Confusion

For decades, lowering cholesterol meant reducing dietary cholesterol — the cholesterol in eggs, shellfish, and organ meats. This guidance has been substantially revised because dietary cholesterol has a relatively modest effect on blood cholesterol levels in most people: the liver adjusts its own production in response to dietary intake. When you eat more cholesterol, most people's livers produce less. The net effect on blood levels is often smaller than the messaging implied. What does affect blood cholesterol more meaningfully — and this took decades to fully appreciate — is dietary saturated fat and trans fat, which influence the liver's production and clearance of LDL through separate mechanisms. The egg, which contains dietary cholesterol, turns out to be a more complex nutritional case than early guidance suggested. The bacon eaten alongside it, high in saturated fat, has a considerably clearer effect on LDL. The foods most reliably associated with cardiovascular risk in rigorous research are not the high-cholesterol foods but the saturated fat-dense and ultra-processed ones — a distinction that, communicated clearly fifty years ago, would have produced different dietary advice and possibly different outcomes.

What Your Number Doesn't Tell You

The standard total cholesterol number tells you approximately as much about your actual cardiovascular risk as your shoe size tells you about your height. It correlates with risk at the population level but is a poor individual predictor — it is possible to have a reassuringly normal total cholesterol while carrying a very unfavourable mix of small, dense LDL particles with high arterial penetration risk, and equally possible to have a technically elevated total cholesterol driven almost entirely by large, buoyant HDL and large LDL particles that carry considerably lower risk. Advanced lipid testing — LDL particle number, LDL particle size, apolipoprotein B — provides meaningfully better risk characterisation than the standard panel. If you have cardiovascular risk factors and a concerning lipid panel, asking your doctor about ApoB in particular is a conversation worth having, because it correlates with cardiovascular events better than LDL-C alone and may change both the assessment of your risk and the urgency of the intervention.

Ask Better Questions Than the Number Answers

Know your full lipid panel, not just total cholesterol. Understand the LDL-HDL breakdown and ask about particle size or ApoB if you have risk factors. Focus dietary changes on saturated fat and ultra-processed food rather than cholesterol-containing whole foods like eggs and shellfish. The number your doctor handed you is a starting point, not a verdict — and understanding what it does and doesn't measure is where the genuinely useful conversation about your heart actually begins.
